We provide responsive, culturally safe support for individuals and families requiring assistance after hospital discharge.
Our team can assist with:

• Community and in-home supports
• NDIS access and coordination
• Linking with allied health and services
• Ongoing support and follow-up
We understand the urgency of discharge planning and aim to respond quickly to referral requests.
We accept self-managed and plan-managed NDIS participants and can also assist individuals not yet connected to supports.
